Due to new Bright From the Start licensing regulations taking place on October 1, 2018, Sisu is requesting that any potential volunteers, interns, therapy-shadow interns, work-learners, nursing instructors, and nursing students please follow the instructions included in the attached fingerprint registration application and complete the process at least 2 weeks before your site work begins.
Classroom Volunteers, Interns, Therapy-Shadowing, & Work-Based Learners:
To become a classroom or facility volunteer you must complete an application packet and attend a volunteer orientation. Classroom volunteers will need to complete a background check, be fingerprinted, and have a TB test. You must by 16 years old to volunteer in a classroom. Please download this form and follow the instructions for fingerprinting. Once finished, please turn in (via email or mail below) the filled out applications and fingerprint registration number to Sisu so we can complete your application process on our side!
Nursing Instructors & Students:

**Please write your designated school in the top right corner of your form (for example: if you attend Clemson University as a nursing student, please write “Clemson Nursing Student” in the top right corner of the form.
